Refer someone to United As 1

Support coordinators, hospitals, allied health professionals, families and other providers can refer someone to us. We'll contact you, and the person, and keep you updated.

Three ways to refer

Whichever suits you

By phone

Call 0406 968 719. We'll take the details and talk through what the person needs.

By email

Email admin@unitedas1.com.au with the person's name, a contact number, and the best time to call. Please don't include health information in an email.

Online, coming with our app

Our secure online referral form keeps the person's information private. It opens with the United As 1 app.

Before you refer

What helps us act quickly

A referral is handing us someone's personal information, so we'll always check that they know.

Good to include

Let the person know you're referring them, and ask for their agreement to share their details with us, if you can.

Tell us how their plan is managed, and who their plan manager or coordinator is.

Tell us what support they're looking for, and the areas they live in.

If it's urgent, say so. We'll call you back the same day where we can.

After you refer

What happens next

1

We call you

We confirm the referral and ask anything we need to know.

2

We contact the person

A friendly conversation about what they're looking for, with no pressure.

3

Intake and agreement

We go through the intake form and service agreement with them, with you involved if they'd like.

4

Support starts

We introduce their worker, and you can follow progress in the app with their permission.
